Nutrition & Food Insecurity
April 1st, 2026, the Idaho WIC program will update our Medical Documentation Form (MDF), due to recent federal WIC food package changes. This form is used when WIC participants require medically needed changes to their WIC food packages or the use of an exempt/therapeutic formula or nutritional. This form is federally required and allows WIC to provide a variety of exempt/therapeutic formulas and nutritional products with the help of your offices. There are two versions of the MDF being shared, a paper and/or a fillable PDF. The updated versions are attached and our website will be updated April 1st, 2026 at https://publicdocuments.dhw.idaho.gov/WebLink/DocView.aspx?id=4461&dbid=0&repo=PUBLIC-DOCUMENTS&cr=1 with these new versions. WIC — The following are acceptable reasons for issuance. Other reasons will be accepted per the healthcare providers clinical judgement.
- Similac Sensitive is a lactose free milk-based formula. It is used for symptoms of fussiness and gas.
- Similac for Spit-Up is a milk-based formula with added rice starch. It is used to reduce excessive spit-up.
- Similac Total Comfort is a milk-based formula that has partially broken-down protein and less lactose for easy digestion. It is used for symptoms of fussiness and gas.
